Truck driver hauling a heavy equipment has a bad day at work

First question is how could this incident have been avoided?

The truck driver thought he was ready to do this misssion. He chained the heavy machine with a chain, and in that tight turn he drove at a slow speed.

But why did that happen? What do you think he did wrong?

Certainly more chains would have been helpful.
